  • Synopsis: The reason for CAPS activation is not yet known. The aircraft landed in hilly terrain near power lines and came to rest inverted. The parachute canopy was draped across a power line tower. High surface winds and mist were reported in the area. The two occupants suffered fatal injuries.

  • Synopsis: Following a go-around due to a traffic conflict, pilot lost control turning to base leg for another landing attempt. ADS-B data detected the airplane slowing to about 80 kts groundspeed just before turning base and ended shortly after the right turn was initiated over Utah Lake near the accident site. A witness saw the airplane’s parachute start to deploy just prior to the airplane impacting the water

  • Synopsis: Pilot and passenger suffered fatal injuries when the pilot flew a non-FIKI aircraft into IMC and forecast moderate Icing over mountainous terrain, the aircraft made a sharp turn, and a rapid descent ensued from which the pilot did not recover. Witnesses reported the aircraft descending out of the cloud base rapidly in a nose down profile, followed approximately six minutes later by an empty parachute.
