As part of the COPA safety initiative, this anthology of NTSB accident investigations presents details behind the NTSB probable cause and factual published reports.
Each NTSB Docket comprises a variety of materials unique to each
accident, typically some ATC radio transcripts, witness and survivor
interview transcripts, radar tracks, wreckage information, NTSB
analysis and reconstruction of the accident flights, photographs of the
accident, and other investigative information. The amount varies considerably between investigations, depending on the circumstances.
COPA requested the public information from the NTSB for all of the Cirrus fatal accidents and parachute activations in their public database. These materials were ordered on-line and arrived on several CDs after about 3-4 weeks. Then using the new capabilities of the COPA 2.0 web site, we uploaded a selected set of them and organized them to make them available to the Cirrus pilot community (see below).
Eventually we hope to assemble this NTSB material for all of the Cirrus accidents. But at present, the upload process is manual. Here is the complete list of docket information.
